진행성 간암 표적치료제 효과 높이는 기전 발견

진행성 간암에서 항암약물 전달 효과를 획기적으로 높일 수 있는 기전이 발견됐다.
 


 
연세대학교 세브란스병원 소화기내과 김도영, 박준용 교수와 의과대학 의과학과 조경주 연구원(박사과정)은 진행성 간암에서 신호전달 단백질 YAP/TAZ의 발현을 조절해 항암약물 전달 효과를 획기적으로 높일 수 있다고 22일 밝혔다. 이번 연구 결과는 미국간학회지 ‘헤파톨로지(Hepatology, IF 17.425)’ 최신호에 게재됐다.

간암은 우리나라 전체 암 중 사망률 2위에 해당하는 난치성 질환이다. 조기에 발견할 경우 치료율이 높지만 진행성 간암의 경우 항암, 방사선, 표적 치료 등에 있어 효과가 상대적으로 낮고 내성이 발생하는 문제로 생존율이 매우 낮다.

간암은 다른 암종에 비해 항암치료제 개발이 느린 편이다. 항암 효과, 이상반응 뿐만 아니라 ‘간 기능’을 함께 고려해야 하기 때문이다. 간의 경우 전체 기능이 악화된 상태에서 암이 발생하는 경우가 많다.

대한간암학회에 따르면 전체 간암 환자의 약 80%는 간경변증을 동반하는 등 여러 기저질환을 갖고 있다. 간경변을 동반한 경우 간에서는 기질이 활성화되고 혈관을 통한 물질전달이 원활하지 않아 항암약물의 치료 효과가 낮아진다.

연구팀은 유전자 조합을 통해 간암에서 나타나는 병리학적 특성을 모방한 마우스모델을 YAP/TAZ의 발현량에 따라 두 모델을 제작해 비교분석했다.

분석 결과 신호전달 단백질 YAP/TAZ의 발현이 낮은 모델에서 암 세포 기질의 활성도가 낮고 약물 투과성이 높게 나타나는 것을 확인했다. YAP/TAZ의 발현이 낮고 기질의 활성이 낮은 모델에서는 시간이 지날수록 약물 투여 효과가 암 주변 조직과 비슷했다. 하지만 YAP/TAZ의 발현이 높은 모델에서는 시간이 지나도 암 조직 내 약물 투과 효과가 암 주변 조직에 비해 약 4배 정도 낮은 것으로 나타났다.
 

 
또한 연구팀은 종양미세환경을 모방한 3D multicellular 간암 오가노이드 모델을 이용해 YAP/TAZ의 발현량에 따른 약물 투과도를 비교 연구를 진행했다. 연구 결과 YAP/TAZ의 발현이 낮은 오가노이드 모델이 암 조직 중심부로의 약물 투과도가 YAP/TAZ의 발현이 높은 모델과 비교해 약물 투과 효과가 약 8배 정도 높게 나타났다.

김도영 교수는 “이번 연구를 통해 간세포암종에서 YAP/TAZ의 발현량의 조절이 암세포 기질의 활성화 정도를 조절하고 약물 투과성을 증가시킬 수 있다는 것을 확인했다”면서 “향후 간암으로의 표적치료제 전달 향상을 위한 임상연구를 계속해 기존 표적 치료의 효과를 높일 수 있을 것으로 기대된다”고 말했다.
